Basal implantology is an advanced and evolving implantology system in which the dental implants utilize the basal bone which is resistant to resorption and a stable part of the osseous tissues of the mandible and maxilla which are underlying the alveolar processes as anchorage for retention and stability. It adopts the concept of “osseofixation” because of its strong anchorage into the cortical bone which can be achieved by utilizing the second or third cortical. This paper provides an extensive overview and a comprehensive knowledge about the concepts of basal implantology and also highlights the available literature regarding the different designs available, their indications, contraindications, advantages, and drawbacks over conventional implants, effectiveness, and their relationships to the long term success of the prosthesis. It also serves as a clinical handbook manual about the armamentarium required and the surgical steps involved in the successful placement of a basal implant. To the best of our knowledge, this is the first of its kind to provide the readers with all the existing literature overview with the manual enclosed for basal implantology
